Wer schwer krank ist, soll seine Behandlung mitbestimmen und seine Werte und Wünsche darin einfließen lassen können. Ist die betroffene Person plötzlich urteilsunfähig, stehen das Behandlungsteam und die Angehörigen vor schwierigen Entscheidungen. Advance Care Planning (ACP), auf Deutsch etwa "vorausschauende Behandlungsplanung", kann solche Situationen klären und zur Entlastung von allen Betroffenen führen. ACP ist ein Werkzeug für PatientInnen, mit dem die eigenen Erwartungen an die Behandlung eindeutig und verständlich schriftlich formuliert werden können. Im Gespräch mit einer Expertin wird eine Patientenverfügung "plus" festgehalten, eine Art erweiterte Patientenverfügung. Dank ihr können das Behandlungsteam und die Angehörigen den mutmaßlichen Willen der betroffenen Person genauer umsetzen.
