A presente técnica cirúrgica consiste em promover uma translação da tuberosidade tibial após uma osteotomia parcial da crista da tíbia, sem necessitar de recorrer à colocação de pinos na tuberosidade tibial, cerclage na crista tibial, ou qualquer método ou instrumento para fixar temporariamente a tuberosidade tibial no seu novo sítio, a fim de verificar o alinhamento e deslizamento da patela antes da fixação definitiva da crista tibial. Para isso foi desenvolvido um instrumento que permite dobrar lentamente a crista tibial parcialmente osteotomizada, conseguindo uma translação da tuberosidade tibial proximal. O instrumento permite mover a parte proximal da tuberosidade tibial tanto para medial como para lateral, mantendo a crista tibial estável na sua nova posição enquanto se confirma que a patela desliza corretamente sobre a tróclea de uma forma estável, antes de fixar definitivamente a crista tibial
Após a transposição da tuberosidade, a crista tibial é fixa com a inserção de um pino entre a tuberosidade tibial e a cortical medial ou lateral da tíbia osteotomizada. O pino tem como objetivo servir de obstáculo (ou calço), impedindo a tuberosidade tibial de voltar à sua posição original como resposta ao efeito memória do osso, até que a consolidação óssea se complete.
